DIRECTIONS FOR FILLING OUT FORMS:

  • Step 1: If you don't have Adobe Acrobat Reader already installed on your computer, you need to download it for free by clicking here: Adobe Reader
  • Step 2: After downloading Adobe Acrobat Reader, click "Download the DLA Application" above.
  • Step 3: A box will come up asking if you want to Open or Save the file. Click on "Save File" then click "OK."
  • Step 4: Choose where to save the file on your computer and click "Save."
  • Step 5: Open the file you just saved with Adobe Acrobat Reader.
  • Step 6: Type in the information on your computer, saving periodically so you don't lose any information.
  • Step 7: When the full application is filled out, save it and close.
  • Step 8: Send the completed application attached to an email to DLAapps@newlifechurch.org
  • Step 9: Pay your application fee (we cannot process your application until your application fee is received. For directions to pay application fee, see below).
  • Step 10: Have your pastor come to this web site and follow step 1-8 to fill out the pastoral recommendation form. Be sure he/she attaches the filled out recommendation form and emails it to: DLAapps@newlifechurch.org.

**You should receive a response from us via email within two weeks of receiving your application. If you do not receive a response from us within two weeks please call us at 719-268-8282.

After we have received your completed application, your application fee, and your pastoral recommendation form we will call you and do an interview. The interview is the final step of the application process but we cannot interview you until all of your forms are in.

DIRECTIONS FOR PAYING YOUR APPLICATION FEE:

Your $50 application fee must be submitted by credit card ONLY. Please e-mail your credit card number and expiration date to DLAapps@newlifechurch.org or call 719-268-8282 and leave your credit card number and expiration date on our secure voicemail.
